We evaluated the effects of trabeculotomy in 77 eyes with glaucoma during the foregoing 10-year period. The series comprised 68 eyes with primary open angle glaucoma and 9 with pseudoexfoliation syndrome. As criteria for entry in the evaluation, trabeculotomy had to be performed as the first surgery and the intraocular pressure could not be controlled under 21 mmHg with topical medication. There was a follow-up period of 3 months or more except in unsuccessful cases. A case was defined as unsuccessful when the intraocular pressure was over 20 mmHg after 2 months, or the mean intraocular pressure for a 3-month period was over20 mmHg even with use of topical medication over a 3-month period, or an additional surgery was required.

A life-table analysis was used in the evaluation of the results. We obtained 49.4% as the probability of 5-year successful control of intraocular pressure for the first operation. There were 7 eyes with complications : hyphema lasting 7 days or more in 4 eyes, detachment of Descemet's membrane in 2, and shallow anterior chamber with intracorneal bleeding in 1. Severe complication following trabeculotomy seemed to be avoidable with care and skill of the surgeon.
