We reviewed the outcome of surgery in 34 glaucomatous eyes. The series comprised primary open-angle glaucoma 31 eyes and capsular glaucoma 3 eyes. Trabeculotomy was performed in 20 eyes and trabeculotomy with phacoemulsification/aspiration and intraocular lens implantation in 14 eyes. The preoperative intraocular pressure (IOP) averaged 20.5 ± 0.86 mmHg in the former group and 20.2 ± 0.90 mmHg in the latter. There was no significant difference between the two groups. At 3 months or later after surgery, the IOP averaged 16.2 ± 0.66 mmHg in the former group and 12.1 ± 0.93 mmHg in the latter. The difference was significant (p =0.0009). Control of IOP below 14 mmHg was attained in 30% in the former group and in 85.7% in the latter. The difference was significant (p = 0.0009) . The findings show the effectiveness of trabeculotomy with simultaneous cataract surgery over trabeculotomy alone. The procedure can be considered in planning surgery for advanced glaucoma.
