眼部MRIにおけるGd-DTPAの造影効果について

Enhancement effect of gadolinium diethylenetriamine pentaacetic acid (Gd-DTPA) in magnetic resonance imaging in ophthalmology

　MRIは有用であるが，病巣部と正常部のコントラストが不鮮明な部分や，病巣内部の性状や広がりのわかりにくい症例がある。眼部諸疾患にGd-DTPAを造影剤として用い，MRIを施行した症例について，その有用性を検討した。対象は眼瞼腫瘍，脈絡膜悪性黒色腫，硝子体出血，裂孔原性網膜剥離，眼窩髄膜腫，球後視神経炎各1例，脈絡膜骨腫の2例8症例7疾患である。全例でMRIによる病巣部の所見が得られ，球後視神経炎の1例を除き，病巣部増強効果が認められた。曖昧な異常陰影をより鮮明にし，病巣部境界を明瞭にするのに役立ち，眼科領域の疾患で，Gd-DTPAを造影剤とするMRI検査は有用であった。

We used gadolinium diethylenetriamine pentaacetic acid, Gd-DTPA, to obtain enhance-ment effects in magnetic resonance imaging (MRI) in 8 patients. The series comprised 2 cases of chor-oidal osteoma and 1 case each of lid tumor, malig-nant choroidal melanoma, vitreous hemorrhage, rhegmatogenous retinal detachment, orbital menin-gioma and retrobulbar neuritis. Increased signal intensities were obtained in all the cases except the one with retrobulbar neuritis with Gd-DTPA at the dosis of 0.1 mmol/kg. This method facilitated dif-ferentiation of ocular lesions from normal tissue.
