超音波生体顕微鏡によるトラベクロトミー術後の隅角観察

Ultrasound biomicroscopy of the chamber angle following trabeculotomy for glaucoma

　隅角全体を断面像として観察することができる超音波生体顕微鏡（ultrasound biomicroscopy：UBM）が開発され，従来は隅角鏡検査のみによって評価されてきた術後の隅角所見を超音波によって垂直断面的にとらえ，術野の強角膜内の変化を観察することが可能となった。そこで今回筆者らは，トラベクロトミー後の隅角をUBMで観察し，隅角鏡検査所見と対比検討した。

　線維柱帯中の裂隙の状態や位置，周辺虹彩前癒着，デスメ膜剥離などさまざまな所見をUBMにて断層像として観察することができた。さらに隅角鏡検査では観察できなかった強膜弁下に存在する間隙や周辺虹彩前癒着に隠された裂隙をとらえることができた。トラベクロトミー後のような複雑な隅角の観察には隅角鏡にUBMを併用することが有用であると思われた。

We observed 44 eyes following trabeculotomy for glaucoma using an ultrasound biomicroscope, UBM. The method gave images of the cleft in the trabecular meshwork, peripheral anterior synechia (PAS) and detached Descemet's membrane. It allowed observation of the space under the scleral flap and the cleft concealed by PAS that cannot be observed by gonioscopy. UBM was thus useful in evaluating the deformed chambe angle following trabeculotomy.
