トラベクロトミー術後の隅角所見の経時変化

Follow up of gonioscopic findings after trabeculotomy

　トラベクロトミー術後の隅角所見の経時変化につき検討した．術後4日〜2週の隅角鏡検査によると虹彩前癒着は術後4日以内に完成されており，それ以降の進行は軽度である．血液逆流は術直後は38％に認めたが急激に減少し，10％前後に落ち着く．したがって隅角所見の基本パターンは術直後には完成されており，裂隙の再閉鎖現象の進行や色素沈着の二次的な修飾をうけると考えられる．

We evaluated the gonioscopic features after tra-beculotomy.In 36 eyes (Group A),gonioscopy was performed 4 to 19 days after surgery.In 64 eyes (Group B),gonioscopy was repeated more than one month after surgery.

Peripheral anterior synechia into the cleft became established within 4 days after surgery. Blood reflux from the clefts was seen in 38% of eyes in Group A,and in about 10% in Group B.

The findings show that the basic gonioscopic pattern is established within a few days after trabeculotomy. This pattern is only slightly modified thereafter by wound healing and pigmen-tation. It was nonproliferative in 24eyes, preproliferative in 6 and proliferative in 5.Retinal vein occlusion was seen in 9 eyes. Photocoagulation was performed to 56 eyes with favorable outcome. The findings point to the impor-tance of detailed fundus examination at an early stage after cataract surgery.
