網脈絡膜剥離と難治性の高眼圧を呈した全身性エリテマトーデスの1例

A case of systemic lupus erythematosus with choroidal detachment and persistent ocular hypertension

　難治性の高眼圧が続いた全身性エリテマトーデスの1例を報告した。症例は55歳の男性で，両眼の結膜充血，右眼の視力低下を主訴に受診した。両眼とも開放隅角で，高眼圧を呈していた。両眼ともargon laser trabeculoplastyにて眼圧は下降せず，1か月後に眼底後極部に網脈絡膜皺襞が出現した。高眼圧が続いたため両眼にトラベクロトミーを行ったところ，術後浅前房となり網脈絡膜剥離をきたした。網脈絡膜剥離は強膜開窓術によって改善したが，トラベクレクトミーなどの降圧手術で再発を繰り返した。再三の降圧手術によっても長期の眼圧下降を得ることはできなかった。

A 55-year-old male presented with bilateral conjunctival hyperemia and decreased vision in theright eye. He had been diagnosed as systemic lupus erythematosus 10 weeks before. Both eyes manifestedelevated intraocular pressure (IOP) and open angle. Argon laser trabeculoplasty was futile. Retinochoroialfolds developed in the posterior fundus one month later. The IOP remained over 30mmHg under maximummedications. Trabeculotomy for both eyes was followed by flat anterior chamber and chorioretinal detach-ment. Sclerectomy induced only temporary reduction of chorioretinal detachment. Further glaucoma sur-geries failed to bring the IOP under control.
