Three cases of orbital myositis developed optic neuropathy. The series comprised a male aged 60 years and two females aged 61 and 64 years. All the cases were unilaterally involved. Decreased visual acuity,chromatopsia,and ocular pain were common complaints. Initial visual acuity was 0.8,0.3,light perception respectively. Diagnostic imaging,including CT and MRI,showed swelling of two or more extraocular muscles. Dysthyroid ophthalmopathy was ruled out by laboratory examinations including autoimmune antibodies. Pulsed corticosteroid therapy follwed by tapering induced remarkable improvement in 2 cases. The final visual acuity was 1.5,1.2,light perception respectively. Optic neuropathy may have been due to compression by swollen extraocular muscles or inflammation of optic nerve. The findings show that optic neuropathy is a liability in orbital myositis or vice versa.
