Gonioscopic findings and effectiveness of trabeculotomy

　トラベクロトミー術後の隅角所見の手術効果への影響を検討した．広範囲癒着型の虹彩前癒着があると長期での手術成績が不良となる傾向を認めた．不鮮明裂隙を有する症例もやや成績不良の傾向を認めた．その他の隅角所見は手術成績には大きな影響を認めなかった．

We evaluated the gonioscopic findings and pres-sure-lowering effect of ab externo trabeculotomy in 96 eyes, including 75 eyes with primary open angle glaucoma and 21 eyes with pseudoexfoliation syndrome. The cases were followed up for at least 6 months after surgery. Cases with premature pene-tration of the probe into the anterior chamber, preoperative peripheral anterior synechia (PAS) or hazy cornea were excluded from the series.

Sufficient control of intraocular pressure resulted in 43 % of the series. The rate of control was lower in eyes with extensive PAS (28 %) than in those without (48 %). Control of intraocular pressure was relatively poor (25 %) in eyes with gonios-copically indistinct cleft. Other gonioscopic fea-tures were not correlated with the surgical out-come.
