びまん性浸潤型網膜芽細胞腫の1例

A case of diffuse infiltrating retinoblastoma

　年長児の片眼にみられ，ぶどう膜炎症状を主体としたびまん性浸潤型網膜芽細胞腫の1例を経験した。症例は6歳，男児。左眼飛蚊症を主訴に来院した。初診時左眼視力（0.5），白色の角膜裏面沈着物，前房cell （3＋），偽前房蓄膿を認めた。虹彩，水晶体上および硝子体に白色の滲出物を認めた。上方網膜に黄白色の滲出斑，網膜血管の拡張を認めた。CT,MRIにて眼内腫瘤が疑われた。硝子体混濁の改善を目的にステロイドパルス療法を施行した。しかし，硝子体混濁は不変かつ白色腫瘤の増大傾向を認めたため，網膜芽細胞腫の診断にて，左眼球摘出術を施行した。術後組織診にて，びまん性浸潤型網膜芽細胞腫と確定した。

A 6-year-old boy presented with seeing flies in his left eye. His corrected visual acuity was 1.5 right and 0.5left. The left eye showed keratic precipitates, 3+ cells in the aqueous, and pseudohypopyon. White exudates were presentover the iris, the lens, and in the vitreous. Funduscopy showed exudates and dilated vessels in the superior quadrant.Computed tomography and magnetic resonance imaging showed intraocular tumorous lesions. Massive systemic corticosteroid failed to improve vitreous opacity. The left eye was enucleated due to suspected retinoblastoma. Histopathologi-cal studies led to the diagnosis of diffuse infiltrating retinoblastoma.
