前房内増殖型網膜芽細胞腫の1例

A case of retinoblastoma with intraocular metastases to the anterior chamber

　12歳の女性に発生した前房内増殖を伴う網膜芽細胞腫の1例を報告した。患者は右眼下方視時の霧視を主訴として来院し，右眼前房内の白色綿花状塊状物と微細な浮遊物，硝子体下方の混濁，下方網膜血管の怒張および鋸状縁付近の白色腫瘤が認められた。視力は両眼ともに1.2（矯正不能）で右眼圧は38mmHgであった。前房水の細胞診で悪性腫瘍細胞が検出され，網膜芽細胞腫と診断され，眼球摘出術が行われた。病理組織学的にはロゼット形成の多い分化型の網膜芽細胞腫で，前房内の塊状物も腫瘍細胞から形成されており，瞳孔を経て前房内に腫瘍が増殖したものと考えられた。眼球摘出後20カ月経過したが再発，転移は認められていない。

A 12-year-old female complained of blurred vision in her right eye, when she angled her head down. Her vision was 1.2 in either eye. In the right eye, the ocular pressure was 38mmHg, and the slit lamp examination showed injection of the bulbar conjunctiva and large aggregates in the anterior chamber. Small tumor masses were found at the inferior fundus. The left eye was normal. A diagnostic puncture of the anterior chamber showed the presence of malignant cells. The right eye was enucleated four months after the onset of symptoms. The pathologic diagnosis was retinoblastoma with rosettes with metastases to the anterior chamber.
