網膜芽細胞腫の予後—眼球外浸潤をみた網膜芽細胞腫10例についての検討

Prognosis of Retinoblastoma with Extraocular Extension

緒言

　網膜芽細胞腫の生命に対する予後は，早期治療が行なわれるようになつて向上してきた^1,2）が，なお不幸な転帰をとるものが存在している。死亡原因としては視神経を介して頭蓋内に転移するものが圧倒的に多いことから，摘出眼球の組織学的検索では，視神経断端部だけの検査が重要視され，他の部分のそれはなおざりにされてきたきらいがある。

　今回私達は，九大眼科で1965年から1974年までの10年間に治療した60例（80眼）のうち，眼球外浸潤をみた10例について検討した結果，興味ある知見をえたので報告する。

Eighty eyes (60 cases) with retinoblastoma were enucleated during the last 10 years (1965 -1974) in Kyushu University Hospital, and ex-traocular extensions of the tumor cells were histopathologically demonstrated in 10 eyes (10 cases). Reviewing these cases, the paper ana-lysed postoperative prognosis of retinoblastoma with extraocular extension.

Invasion of tumor cells in the cut end of the optic nerve was observed in 4 cases, of which 3 died within 2 years after the enucleation.Extension of the tumor cells out of the sclera was observed in another 4 cases, of which 3 died in 2 years after the surgery.
