A 57-year-old male presented with visual loss in his right eye since one month before. He had had untreateddiabetes mellitus since 17 years before. The right eye showed orbital apex syndrome with proptosis, complete ophthalmoplegia and no light perception. No abnormal findings were present in the nasal cavity or paranasal sinus. Computed tomographyshowed orbital mass and pulmonary lesion. We suspected fungal infection after detecting elevated serum level of im-munoglobulin E (IgE) . Resection of orbital mass led to the intraoperative diagnosis of mucormycosis. The surgical areawas irrigated with fluconazol. Biopsy of the lung showed cryptococcosis. Treatment with blood sugar control and systemicfluconazol resulted in improved IgE level and cellular immunity followed by resolution of orbital and pulmonary lesions.It appeared that hyperglycemia and deficient cellular immunity played crucial roles in the mycotic lesions. Elevated serum level of IgE was the initial clue to suspect mycotic nature of the orbital tumor.
