大分医科大学病院眼科において1990年から1999年に施行された全身麻酔下手術633件を対象に,手術患者の特徴を検討した。全身麻酔選択に関して、患者側に何らかの要因があったものは全体の72.9%と,疾患や手術の性質以上にウエイトが大きかった。とりわけ術中患者の協力が得られない小児や知能障害者が多かった。若年者の割合が高く,14歳以下の小児が全体の62.4%を占めていた。緊急手術は全体の11.5%であった。術式では斜視手術が全体の32.4%で最多であった。全体の32.7%に何らかの術前全身合併症があり,全身麻酔のリスクとなる合併症も含まれていた。
We reviewed 633 cases which underwent eye surgery under general anesthesia during the past 10 years.General anesthesia was necessitated in 455 patients (71.9%) by circumstances of the patient including age, mental state,and history of allergy. Out of the 633 cases in the series, 395 (62.4%) were aged 14 years or less and 73 (11.5%) werecases undergoing emergency surgery. Strabismus was the most frequent cause of surgery in 205 cases (32.4%). Systemicproblems were present at the time of surgery in 207 cases (32.7%) , including minor risks for general anesthesia. Thefindings show that general anesthesia had to be performed because of circumstances of the patient rather than the actualcausative disease or type of surgery.
