アトピー性皮膚炎に発症した毛様体突起部無色素上皮剥離の検討

Detachment of nonpigmented epithelium of the ciliary process in patients of atopic dermatitis

（R2-7RM−17）　アトピー性皮膚炎症例に発症した毛様体無色素上皮剥離のうち，毛様体突起部まで剥離がみられた11例11眼の手術成績を検討した。有水晶体眼は8眼で全てに自内障がみられ，偽水晶体眼は2眼，無水晶体眼は1眼であった。有水晶体眼に対しては白内障同時手術を行い，強膜バックリング術または硝子体手術を施行した。初回手術で水晶体切除または水晶体嚢切除を行った4眼では全例で復位が得られた。一方，後嚢を残した7眼のうち5眼は再手術を要し，4眼は水晶体嚢切除を，1眼は毛様体無色素上皮の部分切除を行い，全例で復位が得られた。術中所見では毛様体無色素上皮と水晶体嚢の癒着が6眼でみられ，これが剥離した網膜を牽引固定していた。

We performed surgery on 11 eyes with detachment of nonpigmented epithelium of the ciliary process in 11 patients with atopic dermatitis. Eight eyes had cataract, 2 eyes were pseudophakic and 1 eye was aphakic. All the eyes underwent scleral buckling and/or vitrectomy. Eyes with cataract also underwent cataract surgery at the same time. Retinal reattachment was obtained in 4 eyes treated by lensectomy or excision of lens capsule after the initial surgery. Out of the 7 other eyes with intact posterior capsule, 5 needed additional and successful surgery comprising excision of lens capsule in 4 eyes and partial resection of nonpigmented epithelium of the ciliary body in one eye. On the operation, adhesion between the ciliary epithelium and the lens capsule was present in 6 eyes resulting in fixed traction of the detached retina.
