水晶体偏位，毛様体上皮剥離を伴う網膜剥離について

Retinal detachment associated with lens dislocation and detachment of the pars ciliaris retinae

　水晶体偏位に毛様体上皮剥離を伴う網膜剥離の2症例の概要を報告した。

（1）本症の臨床的特徴として，若年者で鈍力外傷歴のある眼に，水晶体の鼻側偏位と耳側の虹彩・水晶体間に毛様体上皮剥離がみられ，耳側網膜に徐々に進行する扁平な剥離が認められた。

（2）本症の発生機序は鈍力外傷によって水晶体偏位，毛様体小帯の牽引とともに硝子体基底部の牽引が働いて毛様体上皮剥離を生じ，それにひき続いて網膜剥離が起こったものと考えられる。

（3）本症の治療は，毛様体上皮裂隙の完全な確認が困難であり，眼球全周に均等な力を加えて，水晶体偏位，網膜剥離の増強を防ぐとともに，裂隙を完全にblockできるencirclingが適当と思われる。

An atypical retinal detachment developed in two young females following blunt ocular trauma. The condition was characterized by the presence of lens dislocation to the nasal direction, detach-ment of the pars ciliaris retinae connected with the zonula ciliaris and slowly progressive, flat retinal detachment. The ciliary epithelial detach-ment may have been induced by the traction of zonular fibers. It extended posteriorly to result in retinal detachment.
