A 38-year- old male presented with blurring in his left eye. The left eye showed signs of iritis and vitreous opacity. Both eyes showed infiltration along retinal veins. He was treated with systemic corticosteroid under the tentative diagnosis of ocular sarcoidosis. The retinal vasculitis exacerbated followed by signs of dry eye 3 months later. Seven years later, serological studies showed elevated titers of IgG antibodies to viral capsid antigens of Epstein-Barr virus (EBV). He was treated by additional systemic acyclovir. The left eye received vitrectomy. Vitreous specimen showed DNA for EBV. The serum antibody titers gradually decreased associated with cessation of endophthalmitis and good visual acuity. EBV infection was suspected as the underlying cause of bilateral granulomatous uveoretinitis.
