We observed malignant optic glioma in a 76-year-old male. The tumor originated in the left optic nerve just anterior to the optic chiasm. It expanded rapidly anterior and posterior to the ciasm along the left optic tract. The initial symptoms simulated optic neuritis in the left eye followed by rapid and severe visual loss. The tumor further spread to the chiasm resulting in optic atrophy in the right eye and papilledema in the left. T-weighted magnetic resonance imaging (MRI) delineated the tumor and the left optic tract. Malignant optic glioma was diagnosed by intracranial biopsy. Bilateral blindness developed 3 months after his initial visit followed by death another 3 months later. This case illustrates the need of early diagnosis and treatment of malignant optic glioma because of its rapid progression with frequent fatal outcome.
