We reviewed 12 eyes of 12 patients of idiopathic macular hole treated by vitrectomy with intentional removal of internal limiting membrane (ILM). The log equivalent of visual acuity averaged -0.88±0.31 before surgery and -0.39±0.44 after surgery. The surgically obtained ILM showed two types by electron microscopy. One consisted of ILM only and the other contained presumed glial cells also. The posterior surface of ILM appeared coarse suggesting possible damage to the retinal nerve fiber layer. In spite of improved visual acuity, the current surgical method may induce damage to the retina after a long-term course.
