We performed penetrating keratoplasty as an emergency procedure for large corneal perforation in 4 eyes during the past 7 years. The corneal perforation developed secondary to rheumatoid arthritis with corneal involvement in one eye and penetrating keratoplasty in one eye each after herpetic necrotizing keratitis, infection after penetrating keratoplasty and systemic thermal burn involving the cornea. The interval between corneal perforation and keratoplasty ranged from 4 to 16 days. The graft remained clear in one eye after 27 months and slightly opaque in 2 eyes. The graft was a failure in one eye due to herpetic endotheliitis with bullous keratopathy. The cases illustrate the efficacy of penetrating keratoplasty for large corneal perforation in preserving the eye. Management of postoperative glaucoma was essential for fair visual outcome.
