レーベル病と多発性硬化症が合併した1例

A case of Leber's hereditary optic neuropathy associated with multiple sclerosis

　レーベル病と多発生硬化症の合併例を経験した。症例は18歳，女性。ミトコンドリアDNA解析で11778変異を認め，レーベル病と診断した。病初期の片眼性の視力低下に副腎皮質ステロイド薬が著効し，病状には増悪と寛解がみられた。両眼性の視力低下を発症してから，副腎皮質ステロイド薬への反応は悪くなった。MRI検査で，視神経および視交叉に脱髄所見があった。本症例は，レーベル病としては非典型的であった。経過中，MRI検査で脊髄にも脱髄病変を確認し、多発性硬化症とも診断した。本症例は，多発性硬化症による球後視神経炎が初めに発症し，その後レーベル病が発症した。

We diagnosed a 18-year-old female as Leber's hereditary optic neuropathy (LHON) . Mitochondrial DNA analysis showed 11778 mutation in this patient, her mother and elder brother. She developed clinical manifestations of multiple sclerosis 2 years later. She showed remissions and relapses in visual disturbance. Magnetic resonance imaging (MRI) showed demyalinating lesions in the optic nerves and thoracic spinal cord.
