3460遺伝子変異をもつレーベル視神経症の視機能回復

Visual recovery in a case hereditary optic neuropathy of Leber with 3460 mutation

　ミトコンドリアDNA （mtDNA）の3460変異をもつレーベル視神経症の症例において，視力の自然寛解を経験し，視力回復後の視機能について評価した。症例は14歳男性，両眼の視神経炎として当院へ紹介され，レーベル視神経症を疑いmtDNAを検索し，3460変異（＋）であった。発症後約1年より次第に回復し，視力が右眼1.2，左眼O．9まで回復した。しかし，視力回復後む色覚，フリッカー値には回復なくゴールドマン視野でも中心暗点を残していた。ハンフリー視野では絶対暗点のなか限局した範囲での回復があり，SLOによるスコトメトリーでも絶対暗点に囲まれたなかに中心窩を含む回復を確認できた。絶対暗点と回復した範囲で検眼鏡的な差異は認められなかった。

A 14-year-old boy developed painless bilateral loss of central vision. When seen one month after onset, he was diagnosed as hereditary optic neuropathy of Leber. Analysis of mitochondria showed 3460 mutation. The visual acuity recovered spontaneously one year later to 1.2 right and 0.9 left. Even then, the color vision was markedly impaired with large central scotoma by Goldmann perimetry. There was sparing of central vision by Humphrey perimetry and by scotometry with scanning laser ophthalmoscope. Ophthal-moscopy showed no differences between the recovered and nonrecovered retinal areas.
