We treated 11 eyes with cataract and with-the-rule astigmatism of 1.0 D or greater by limbal relaxing incision and phacoemulsification through a temporal clear corneal small incision. The preoperative corneal astigmatism averaged 2.0 ± 0.8 D. The refractive cylinder 3 months after surgery averaged 0.5 ± 0.5D. The corneal endothelial cell loss averaged 9.2%. All the eyes achieved corrected visual acuity of 20/20. There was no complication including axial rotation, overcor-rection or corneal perforation. The present combined technique was thus safe, easy and effective for cataract with pronounced with-the-rule astigmatism.
