難治性眼サルコイドーシスの2症例

Two cases of recalcitrant ocular sarcoidosis

（17-P2-9）　23歳男性と69歳女性が眼サルコイドーシスによる網膜ぶどう膜炎で受診した。第1例，第2例とも両眼発症であった。副腎皮質ステロイドの局所と全身投与で炎症は寛解したが，高度の硝子体混濁，血管新生，硝子体出血が続発した。第1例では網膜剥離により視力が失われた。第2例では，硝子体手術により視力の改善が得られた。両症例とも当科受診前に，それまでの治療を長期間中断していたことが重症化の一因であると推定された。

A 23-year-old male and a 69-year-old female presented with severe uveoretinitis as manifestation of ocular sarcoidosis. Both eyes were involved in both cases. Massive vitreous opacity, neovascularization and vitreous hemorrhage developed after initial improvement of retinouveitis by topical and systemic corticosteroid. The vision in the affected eye in the first case was lost due to retinal detachment. Improvement in visual acuity resulted after vitrectomy in the second case. Longterm discontinuation of treatment before visiting us appeared to be the major cause of aggravation of ocular sarcoidosis in both cases.
