急性リンパ性白血病に合併したサイトメガロウイルス網膜炎の1例

Cytomegalovirus retinitis in a case of acute lymphatic leukemia

（17-P1-9）　22歳女性が急性リンパ性白血病のために骨髄移植を受けた。その148日後に左眼の中心暗点が突発した。眼底所見と，前房水のPCR法による検査の結果両眼のサイトメガロウイルス網膜炎と診断した。骨髄機能が低下していたので，フォスカーネットの全身投与を行ったが無効であった。さらに病変が進行したので，網膜光凝固術とガンシクロビル硝子体内注入とを行った。右眼の網膜炎は沈静化した。左眼に続発性網膜剥離が生じたが，硝子体手術とシリコンオイル注入で復位が得られた。サイトメガロウイルス網膜炎で迅速な治療開始が必要なことを示す1例である。

A 22-year-old female underwent bone marrow transfusion for acute lymphatic leukemia. She noted sudden central scotoma in her left eye 148 days later. Both eyes were diagnosed as cytomegalovirus retinitis on funduscopic findings and examination of aqueous humor processed by polymerase chain reaction (PCR). Because of bone marrow suppression, she was initially treated by intravenous foscarnet which proved futile. She was then treated by retinal photocoagulation and intravitreal ganciclovir. The retinitis in the right eye was arrested. The left eye developed secondary retinal detachment which was cured by vitreous surgery with silicone oil tamponade. This case illustrated the necessity of prompt treatment for cytomegalovirus retinitis.
