特発性脈絡膜新生血管に対する黄斑下手術の成績

Outcome of surgical removal in idiopathic choroidal neovascularization

（P-2-129）　新生血管膜が1乳頭径を超える特発性脈絡膜新生血管（idiOpathrc choroldal neovascularization：lCNV）に対し，1998年11月から1999年2月の間に新生血管膜摘出術を4例4眼に行った。発症年齢は平均37歳，全例女性であり，屈折誤差は＋0.5から−3.5Dであった。術前矯正視力は全例0.1以下であり，脈絡膜新生血管膜の大きさは平均1.93乳頭径（1.62から2.24）であった。視力の改善が得られた症例は3例で，不変は1例であった。また術後の絶対暗点面積は，全例で術前と比較して縮小した。脈絡膜新生血管が大型で，術前の視力が悪いICNVに対しては，黄斑下手術が有効な治療法の1つになると思われた。

We surgically removed the coroidal neovascular membrane in 4 eyes of 4 patients with idiopathic choroidal membrane. Their ages ranged from 31 to 42 years, average 37 years. The refraction ranged from + 0.5 to -3.5 diopters (D) , average -1.5 D. The initial corrected visual acuity was 0.1 or less in all the eyes. The size of neovascular membrane ranged from 1.62 to 2.24 disc diameters (DD), average 1.93 DD before surgery. After surgery, the visual acuity improved by 2 lines or more in 3 eyes. Absolute scotoma decreased in size in all the 4 eyes. The findings show that vitreous surgery may be useful in the treatment of eyes with idiopathic choroidal neovascularization.
