収縮した残存水晶体嚢に眼内レンズが完全に包まれた2症例

Two cases of intraocular lens wrapped by contracting remnant lens capsule

（18-P2-1）　白内障術後に，著明に収縮した水晶体嚢に眼内レンズ（intraocular lens：IOL）が完全に包まれた2症例を経験した。症例1では，CCC （continuous curvilinear capsulorhexis）が6mmで90°の毛様体小帯の断裂を認め，術後2年で残存水晶体嚢が著明に収縮し，IOLが嚢に完全に包まれた（以下，前嚢の完全収縮と呼ぶ）。症例2では，術前から180°の毛様体小帯の断裂を認め，術中CCCが4mmで術後わずか2週間で前嚢の完全収縮を認めた。2症例とも，強烈な嚢の収縮にもかかわらず，PMMA製1ピースレンズの光学面は平行性を維持しており，YAGレーザーによる前嚢・後嚢切開術により良好な視力を得ることができた。

We observed 2 eyes in which the intraocular lens (IOL) became completely covered by contracting remnant lens capsule. In one eye, the initial surgery had been performed by continous curvilinear capsulorhexis (CCC) 6 mm in diameter with dialysis of zonules over one quadrant. When seen 2 years later, the IOL placed in the bag had become completely wrapped by contraction of the anterior capsule. The another eye had been treated by CCC 4mm in diameter with previous dialysis of zonules over two quandrants. Complete contraction of anterior capsule developed 2 weeks later. In both eyes, the IOL remained parallel to the iris plane. Full visual acuity was obtained in both eyes after YAG laser capsulotomy.
