後房レンズにプロペラ現象を認めた1例

A case of propeller phenomenon of implanted posterior chamber lens

（展示80）　眼内レンズのプロペラ現象は，前房レンズの開発初期に，前房内で固定不良の眼内レンズの光学面が視軸回りに回転することから名付けられた。アトピーの27歳男性に白内障手術を行い，嚢外移植した後房レンズに典型的なプロペラ現象が術直後から生じた。視力は良好であったが，プロペラ現象による複視・グレアなどに対する不満が強く，より大きな後房レンズに入れ替える手術を施行した。再手術後の視力は良好で，プロペラ現象による合併症はすべて消失した。

Propellering of intraocular lens (IOL) used to be a common complication during the early stages of developmen of anterior chamber IOL. We observed this phenomenon in a 27-year-old atopic male who underwent cataract surgery with posterior chamber lens implantation fixated out of the bag. While the postoperative visual acuity was satisfactory, the patient complained of diplopia and glare due to propellering of IOL. We exchanged the posterior chamber IOL for a larger one. After this procedure, all the complications due to propellering disappeared with good visual acuity.
