原因不明の硝子体出血に対する硝子体手術

Vitreous surgery for vitreous hemorrhage of unknown origin

（18-P1-43）　原因が不明な硝子体出血に対して硝子体手術を行った15眼を検討した。術後に判明した原因疾患は，網膜静脈閉塞症10眼，裂孔原性網膜剥離2眼，老人性黄斑変性症1眼，網膜細動脈瘤1眼，ぶどう膜炎1眼であった。網膜剥離2眼では手術後に増殖性硝子体網膜症が進行して剥離が再発し，硝子体手術を追加して復位が得られたが，視力転帰は不良であった。網膜静脈閉塞症眼の視力転帰は比較的良好であり，特に網膜静脈分枝閉塞症眼では手術時期が早いほど一般に良い視力が得られた。

We reviewed 15 eyes which underwent vitreous surgery for idiopathic vitreous hemorrhage. After surgery, the causative lesions were identified as retinal vein occlusion 10 eyes, rhegmatogenous retinal detachment 2 eyes and one eye each of age-related macular degeneration, retinal macroaneurysm and uveitis. The 2 eyes with retinal detachment developed proliferative vitreoretinopathy necessitating additional surgery. The visual outcome in these 2 eyes was poor in spite of retinal reattachment. Eyes with retinal vein occlusion resulted in favorable visual outcome, particularly in eyes with branch retinal vein occlusion when treated early.
