We surgically treated the left eye of a 60-year-old female with chronic myelogenous leukemia. Total retinal detachment had persisted for over three months secondary to massive subretinal hemorrhage. The visual acuity was no light perception at the time of surgery. Closed angle glaucoma was present due to anterior displacement of the iris and the lens. The retina was visible posterior to the lens. We drained a copious amount of tar colored subretinal blood. After closed vitrectomy following the first subretinal tap, the retina was apparently reattached. The intraocular pressure normalized thereafter. Subsequent vitrectomy for massive vitreous hemor-rhage resulted in improvement of vision to counting fingers. This case illustrates that total retinal detachment with massive subretinal hemorrhage may successfully be treated by adequate surgical means.
