先天性白内障手術後におこる網膜剥離の検討

Retinal Detachment after Congenital Cataract Surgery

緒言

　先天性白内障の手術を受けたことがある眼に起こつた網膜剥離（Aphakic Retinal Detachmentafter Congenital Cataract Surgery，以下ARDCと略）は，それほど多いものではない。しかし，その取り扱いは難しく，また手術予後も一般には良くない。老人性白内障手術後に起こる網膜剥離（Aphakic Retinal Detachment after Se—nile Catarct Surgery以下ARDSと略）は，術後1年以内に起こるのが多いのに反して，ARDCは，術後10年以上の長年月を経て起こる傾向にあることが臨床上の経験で知られている。そこで，この種の網膜剥離の特徴を分析することは，未だ不明の網膜剥離の発生機序の解明や，予防および治療面の改善からも必要となつてくる。

　ARDSを主体としたものの分析は数多くみられる。しかし，先天性白内障術後のものだけを取り上げたのはCordes^1）による54眼の組織学的検索が見られるのみで，臨床面を分析したものはないようである。本編では，純粋にARDCのみ114眼を取り上げ，これらの臨床所見を分析し，その病因をさぐるのを主眼とした。なお，治療については別の編で述べる。

It is known that the retinal detachment oc-curring after congenital cataract surgery is difficult to manage and has poor surgical prog-nosis. One hundred and fourteen eyes with this condition seen at the MEEI, Boston, were analysed and an attempt was made to elucidate its pathogenesis and to find some advancement in managing these eyes.

The major characteristic features of these eyes were a high incidence of males and my-opia and common occurrance in the fourth and fifth decades of life (32 years of age in average) with a long interval after cataract surgery (23 years in average).
