黄斑円孔を伴う網膜剥離の手術法の改良と視力予後について

Improvements of surgical procedure for retinal detachment with macular hole and post-operative visual acuity

　黄斑円孔を伴う網膜剥離の，黄斑プロンベを用いる手術法を一部改良した。その結果，手術成績が向上し，術後の視力もよいものが多くなった。主な改良点は，

（1）4-0 Supramid糸あるいはDacron糸を，上・下斜筋腱付着部間強膜に，角膜輪部より等距離に幅7mmでZ字通糸する。

（2）黄斑プロソベの挿入路を、双眼倒像検眼鏡にて観察しながら，直視下で確実に作る。

（3）網膜下液を除去後あるいは，網膜剥離が後部葡萄腫の範囲に限局している場合には前房水を少し漏らし，限圧を低下させてからプロンベを挿入，縫合糸をしめて固定する。プロンベ後端の縫合固定は不要である。

（4）プロンベ先端のbuckling部位置の左右への移動は，プロンベを軽く振ることにより可能である。

（5）ジアテルミー凝固，冷凍凝固はもとより，光凝固も不要である。

The surgical results and post-operative visual acuity in 40 eyes of 39 patients suffering from retinal detachment with macular hole were studied. Thirty-six out of 40 eyes were operated on by a buckling procedure using a macular plomb de-veloped by the author.

In 33 out of 36 eyes, reattachment of the retina were obtained successfully. The post-operative visual acuities in the later period in which the improvements were made on a few points of surgi-cal procedures, were superior on an average to those of the earlier period.
