黄斑部に及んだ網膜剥離の術後視力

Postoperative visual acuity of rhegmatogenous retinal detachment with macular involvement

　網膜剥離が発症する前は正常な視力があったと考えられる症例で黄斑部に剥離が及んだ113例113眼について復位後の視力を検討し，次の結果を得た。

　（1）術後，113眼のうち104眼（92％）に視力の改善がみられ，65眼（57.5％）が0.5以上の視力に改善した。

　（2）黄斑部の剥離の程度と術後視力の間には強い関係がみられた。

　（3）黄斑部の剥離の高さが4〜9Dの扁平な症例では72眼中36眼（50％）が12カ月目およびそれ以降にも視力の改善がみられ，しかも術後視力0.5以上の症例が36眼中30眼（83.3％）もあった。

　（4）黄斑部の剥離の高さが10D以上の症例，脈絡膜剥離を合併した症例，6カ月以上黄斑部が剥離していた症例では約半数が術後1カ月以内で視力が固定し，0.5以上の視力に改善した症例はほとんどなかった。

A retrospective survey was made on 113 eyes with rhgematogcnous retinal detachment with par-ticular attention to factors that might affect post-operative visual acuity. The interval between detachment surgery and the present evaluation ranged from 13 months to 5 years and 9 months. Only such eyes were selected in which the macula was detached prior to surgery and was reattached at the first operation. Preexisting macular affections, high myopia, opacities in the ocular media, glau-coma, postoperative macular hole or macular puck-er were excluded from the survey.
