輪部腫瘍が疑われたハンセン病の1例

Presumed limbal tumor in a case of leprosy

（26E-17）　角膜に接した上強膜の隆起と角膜混濁で発症し，ハンセン病と診断した未治療の症例を経験した。症例は22歳で，ミクロネシア連邦出身，両眼の充血と眼痛で発症した。瞼裂部上強膜の充実性腫脹と充血，隣接する角膜には実質深層の混濁や浸潤があり，特に左眼で著明であった。0.1％ペタメタゾン，0.3％オフロキサシン点眼で数か月加療したが改善しなかった。顔面や両手背などに皮膚結節があり，皮膚科で手背皮膚結節の生検からIepromatous leprosyと診断された。ジアミノージフェニルスルホン内服を開始したところ，皮膚病変，眼所見ともに軽快した。

A 22-year-old Micronesian woman presented with redness and pain in both eyes. She showed pink episcleral nodules on both sides of the exposed corneoscleral limbus. The peripheral cornea adjacent to the nodules showed diffuse opacities with punctate lipid infiltration. She had numerous nodular lesions on the face and the hands. Skin biopsy showed foamy cells and acid-fast bacilli which were identified as Mycobacterium leprae, leading to the diagnosis of lepromatous leprosy. The sclerocorneal lesions failed to respond to topical treatment with ofloxacin and betamethasone and regressed after oral treatment with diamino-diphenyl sulfone.
