ポリープ状病変の自然消失をみたidiopathic polypoidal choroidal vasculopathyの1例

Spontaneous resolution of polypoidal lesions in a case of idiopathic polypoidal choroidal vasculopathy

　経過観察中にポリープ状病変が自然消失したidiopathic polypoidal choroidal vasculopathyの1例を報告した。患者は72歳，男性。左眼視力低下を主訴に来院。右眼視力（0.04），左眼視力（0.8）。左眼黄斑部には出血性色素上皮剥離があり，鼻下側辺縁部の橙赤色病巣を光凝固し，色素上皮剥離は消失した。右眼には角膜片雲があり，黄斑耳側に橙赤色病巣がみられ，赤外螢光造影で分岐する脈絡膜血管網とポリープ状病変が認められた。10か月後の造影所見でポリープ状病変は自然消失していた。右眼のポリープ状病変の消失は，血管網と交通する脈絡膜血管の閉塞によると推測された。

A 72-year-old male presented with blurring of vision in the left eye. Funduscopy showed hemorrhagic detachment of retinal pigment epithelium (RPE) in the left macula. Photocoagulation of suspected neovasculariza-tion at the edge of detachment induced resolution of RPE detachment. An orange-red nodular lesion was present temporal to the right macula. Indocyanine green angiography showed a choroidal vascular network with polypoidal lesions. These lesions resolved spontaneously ten months later. It appeared that the resolution of polypoidal lesions was due to occlusion of choroidal feeder vessels.
