We reviewed the fate of 257 cases of initially unilateral senile disciform macular degeneration (SDMD). The patients were followed up for 1 to 21 years, average 5 years 3 months. During the course of observation, choroidal neovascularization (CNV) developed in 35 fellow eyes. The risk of developing bilateral CNV was calculated to be 0% at 1 year, 6.6% at 3 years and 12.6% at 5 years. Pigment epithelial detachment (PED) was the most commonly identified lesion predisposing to CNV. Fluorescein angiography in these eyes showed both classical and irregular filling pattern in the PED. The findings show that the presence of PED in the fellow eye of SDMD indicates the need of close observation.
