老人性円板状黄斑変性症の光凝固長期の治療成績

Longterm results of photocoagulation for senile disciform macular degeneration

　老人性円板状黄斑変性症に，クリプトン・レーザー，および色素レーザーを用いて脈絡膜新生血管に対する光凝固治療を行い，光凝固後1年以上経過観察を行った135例142眼の治療後長期の成績を調べた。治療効果を，視力と眼底所見を総合してみると，全体の55％に改善がみられた。今までに報告した光凝固後1年以内の成績と比べて各群に成績の低下をみ，新生血管が中心窩から200μm以内と近かった症例に新生血管の再発をみた例が多かった。本症早期の漿液性網膜剥離期の症例の改善率は70％であった。本症早期に十分な光凝固を行うと長期問経過してもよい成績が得られた。

We treated 142 eyes with senile disciform macular degeneration with krypton red or organic dye laser photocoagulation. All the cases were followed up for 1 year or longer. The laser treat-ment was effective for the disease in its early stage manifesting serous retinal detachment with tinyhemorrhage or exudation before onset of disciform lesion. The treatment was effective in 74% of eyes in this stage. The rate of success decreased after 1 year or longer. Choroidal neovascularization was prone to recur when the initial lesion was located within 200μm from the fovea. We advocate photocoagulation during the early stage of the disease with long-term follow up after treatment.
