We evaluated the clinical features of 45 cases with idiopathic senile macular hole either in its manifest or incipient stage. Manifest macular hole present in 40 eyes, of which 4 cases were bilateral. Suspicious but less typical macular lesion was seen initially in 9 eyes in 9 cases. The followup period ranged from 3 to 84 months and averaged 29 months.

Manifest macular hole developed in 4 of 7 eyes (57%) with macular cyst and 1 of 3 eyes with navel-like macular lesion during the course of observation. Posterior vitreous detachment apparently contrib-uted to the formation of macular hole in 1 of these 5 eyes. The presence of macular cyst thus seemed to be the most likely prodromal affection leading to idiopathic macular hole. Occurrence of posterior vitreous detachment would act as a trigger to make the macular hole manifest.

In 3 eyes, the macular hole became less conspic-uous either due to reattachment of the opeculum or to tissue proliferation within the hole.

The 24-hour estrogen level in the urine was quan-titated in 19 female cases with unilateral or bilateral macular hole. The value was within normal range in 13 cases.
