A 25-year-old male presented with diplopia of one week's duration. Disc edema was found in the left eye. He was treated by antibiotics and corticosteroids. Transnasal ethmoidectomy was performed under suspicion of rhinogenous optic neuropathy. He then developed ipsilateral pupil disorder, restricted eye motility, lid ptosis, exophthalmus and facial palsy. Malignant lymphoma was detected by further endonasal ethmoidectomy. Chemotherapy and radiotherapy were futile. Paranasal lymphoma is a possibility in the differential diagnosis of rhinogenous optic neuropathy with multiple cranial nerve involvements.
