篩骨洞に発生した真珠腫の1例

A Case of Cholesteatoma in the Ethmoid Sinus

　はじめに

　真珠腫（cholesteatoma）は，先天性あるいは炎症性などに起因する中耳角化性病変として耳鼻咽喉科医に広く認識されている。したがって，副鼻腔に発生する真珠腫は臨床的に経験する機会は少なく，これまで本邦では8例しか報告されておらず，しかも篩骨洞の真珠腫例は海外の2例以外はない。今回われわれは，眼球突出により発見された極めて稀な篩骨洞原発の真珠腫の1例を経験したので，成因などについて若干の文献的考察を加えて報告する。

A 52-year-old man presented with cholesteatoma in the posterior ethmoid sinus, which caused exo-phthalmus.

Only two reports of cholesteatoma of the ethmoid sinus were found in foreign literatures, none in Japanese. A part of the capsule of cholesteatoma was removed and the cavity was widely opened into the nasal cavity, but total extirpation was not car-ried out. The origin of cholesteatoma was supposed to be embryonic mesenchym or squamous meta-plasia.
