健常人に発症したサイトメガロウイルス網膜炎の1例

Cytomegalovirus retinitis in a healthy person

（18P1-11）　症例は66歳の男性。右眼飛蚊視を主訴に当科を受診した。初診時，右眼鼻上側周辺部に限局性の滲出斑と軽度斑状出血がみられた。やがて滲出斑は増大し，白線化した血管もみられ，炎症所見は増悪した。当初桐沢型ぶどう膜炎を疑い，アシクロビルなどの投与を開始したが，採取した前房水のPCR法によるウイルスDNA検索の結果，サイトメガロウイルスが検出された。ガンシクロビル投与に切り替え，炎症所見は鎮静化した。全身検査の結果，免疫不全を示唆するものはなかった。ガンシクロビル投与中止後9か月経過した現在でも再燃はなく，良好な視力を維持している。

A 66-year-old male presented with floaters in the right eye since one week before. The affected eye showed localized exudates with hemorrhagic patches in the superior nasal quadrant. The lesion gradually enlarged associated by vascular occlusions. The findings were suggestive of acute retinal necrosis. Analysis of aqueous tap by polymerase chain reaction (PCR) showed cytomegalovirus. Systemic ganciclovir induced remission of inflammatory process. Signs of im-munodeficiency were consistently absent. The patient is doing well with full visual acuity during 9 months after discontinu-ation of ganciclovir.
