A 36-year-old male underwent penetrating keratoplasty on his left eye for corneal perforation after repeated episodes of uveitis, keratitis and bacterial corneal infection. Bullous keratopathy developed due to rejection of the graft. His right eye became blind due to neuroretinitis. The left eye then underwent autokeratoplasty from the right eye. The right eye was treated by keratoplasty using a donor cornea. The autografted left eye maintained corneal transparency. The homografted right eye developed two episodes of rejection. The endothelial cell density in the left eye decreased to 48.5% one year later from the level immediate after autokeratoplasty to remain constant thereafter. The endothelial cells in the right eye remained stable for 6 months after homokeratoplasty to decrease rapidly after onset of rejection. The findings show that the decrease in endothelial cell density in the autografted left eye was due to the endothelial migration from the center to periphery.
