網膜剥離失明眼の晩期合併症

Late complications in unhealed retinal detachment

　未治療もしくは非治癒の網膜剥離眼に長期間を経て発症した合併症につき検討を加えた。最近3年間に6例の症例がみられ，網膜剥離の持続期間は8年から60年にわたっていた。3例は未治療であり，3例は手術を受けたが非治癒もしくは再剥離をきたした症例であった。5例は緑内障を起こしており，白内障，虹彩後癒着，周辺虹彩前癒着が共通してみられ，また4例にはiris bombéがみられた。眼圧上昇の機序としては周辺虹彩前癒着もしくはiris bombéが関与していると思われた。未治療もしくは非治癒の網膜剥離眼は，たとえ失明しているといえども定期的な経過観察が必要であると考えられた。

We reviewed 6 eyes of unhealed retinal detachment regarding late complications. The duration of detachment ranged from 8 to 60 years. No surgery had been performed in 3 eyes. Detachment did not heal or recurred after surgery in 3 eyes. Five eyes manifested glaucoma, cataract, posterior synechia and peripheral anterior synechia in common. Iris bombé was present in 4 eyes. The elevated intraocular pressure appeared to be due to peripheral anterior synechia or iris bombé. The findings point to the importance of longterm follow-up in eyes with persistent retinal detachment.
