中毒性表皮壊死症による角結膜障害の1症例

A case of toxic epidermal necrolysis with corneoconjunctival injury

　36歳男性の左眼の網膜中心静脈閉塞症に続発した血管新生緑内障に対して毛様体冷凍凝固術施行後，中毒性表皮壊死症が発症した。発症初期よりドライアイ，輪部結膜壊死，角膜上皮欠損がみられ後に角膜帯状変性を併発，左眼は角膜穿孔をきたし，右眼は遷延性角膜上皮欠損となり，最終的には角膜は結膜で被覆された。本疾患の角結膜障害は，全身の表皮壊死融解とともに眼表面上皮が壊死し，それに伴い幹細胞が疲弊するためと推定された。本症例での発症の原因は不明であった。

Toxic epidermal necrolysis developed in a 36-year-old male following cyclocryotherapy for rubeotic glaucoma in the left eye. Both eyes developed dry eye, necrosis of limbal conjunctiva, corneal epithelial defect and band keratopathy. Perforation of left cornea and persistent epithelial defect of right cornea healed by covering with the conjunctival flap. The ocular lesions seemed to have resulted from destruction of corneal stem cells in the limbal area by pathologic process of toxic epidermal necrolysis.
