A 79-year-old male was referred to us for failing vision in both eyes. He was diagnosed as subcapsular and cortical cataract in the right eye and hypermature cataract in the left. Both eyes showed arcus senilis and narrow chamber angle. Prophylactic argon laser iridotomy in the left eye was followed by phacolytic glaucoma the next day. It was probably caused by locating the iridotomy near the pupillary margin due to the arcus senilis. Iatrogenic defect in the anterior lens capsule would have induced flow of phacolytic material into the anterior chamber. We advocate due precaution in prophylactic laser iridotomy by proper selection of eyes, careful laser procedures and detailed posttreatment follow-up.
