アルゴンレーザー虹彩切開術後に生じた水晶体融解緑内障の1例

A case of phacolytic glaucoma following argon laser iridotomy

　症例は79歳の男性で，10年来左眼視力は不良であり，1週間前から右眼霧視が出現したため当科を受診した。両眼とも老人環があり，狭隅角で，右眼は後嚢下および皮質白内障，左眼は過熟白内障であった．左眼の予防的アルゴンレーザー虹彩切開術後，水晶体融解緑内障を発症した。老人環のため虹彩切開部が瞳孔縁寄りになったこと，過熟白内障のため融解した水晶体皮質が微細な前嚢の損傷部から前房内へ流出したことが，発症原因と考えられた。レーザー虹彩切開術後，まれに重篤な合併症を生じることがあるので，症例の選択，手術手技，術後の経過観察などには十分な注意が必要である。

A 79-year-old male was referred to us for failing vision in both eyes. He was diagnosed as subcapsular and cortical cataract in the right eye and hypermature cataract in the left. Both eyes showed arcus senilis and narrow chamber angle. Prophylactic argon laser iridotomy in the left eye was followed by phacolytic glaucoma the next day. It was probably caused by locating the iridotomy near the pupillary margin due to the arcus senilis. Iatrogenic defect in the anterior lens capsule would have induced flow of phacolytic material into the anterior chamber. We advocate due precaution in prophylactic laser iridotomy by proper selection of eyes, careful laser procedures and detailed posttreatment follow-up.
