後房レンズ毛様溝縫着術後に生じた網膜剥離の2症例

Two cases of retinal detachment following posterior chamber lens implantation sutured to the ciliary sulcus

　後房レンズ毛様溝縫着術後，網膜剥離を生じた2症例について報告する。2症例は後房レンズ毛様溝縫着術後，2年と半年で網膜剥離を発症した。硝子体手術を行った症例2においては，後房レンズ支持部縫着部位より近接した網膜裂孔への硝子体牽引が確認された。これは縫着の通糸，または縫着の操作により，硝子体線維の変性，増殖組織の発生が誘発され，硝子体牽引を惹起し，裂孔原性網膜剥離を発症させたものと思われた。

　従来，後房レンズ毛様溝縫着術後の網膜剥離の報告例は少ないが，今後，同様の症例の発生の増加が予想されるため，十分な注意が必要と思われた。

Retinal detachment developed in two pseudophakic eyes of two patients. These eyes had received posterior chamber lens implantation sutured to the ciliary sulcus 2 years and 6 months before respectively. Retinal tear with vitreous traction was detected near the site of suture in one eye during vitreous surgery. Surgical manipulation during suturing appeared to have induced the retinal tear and vitreous traction.
