A 69-year-old male with noninsulin dependent diabetes mellitus had been treated by panretinal photocoagulation in both eyes for preproliferative retinopathy 3 years before. When the HbA1c level was reduced from 11.3% to 8.6% during 26 days by intensified insulin therapy, the right visual acuity decreased from 0.7 to 0.15 due to exudative macular detachment. The left visual acuity deteriorated from 1.0 to 0.2 due to similar macular lesion 70 days after start of insulin. Grid photocoagulation improved the visual acuity to 0.5 in both eyes. This case seemed to illustrate that acute macular lesion is a liability after drastic insulin therapy.
