メチシリン耐性黄色ブドウ球菌による術後眼内炎の1例

A case of postsurgical endophthalmitis by methicillin-resistant staphylococcus aureus

　Methicillin Resistant Staphylococcus Aureus （MRSA）による白内障手術後の眼内炎の1例を経験した。患者は80歳女性で，白内障手術後9日目から眼内炎をきたし，翌日眼内レンズ（IOL）摘出術および硝子体手術を施行した。術前採取した前房水，術中採取した前房水，後嚢，硝子体の培養からMRSAが検出され，感受性のある抗生物質の局所投与および全身投与により眼内炎は治癒した。また，術中摘出したIOLの表面を，電子顕微鏡にて観察したところ，球菌がIOL表面細胞中に多数貪食されているのがみられ，培養結果を踏まえるとそれは貪食されたMRSAと考えられた。

An 80-year-old woman developed endophthalmitis 9 days after extracapsular cataract extrac-tion and intraocular lens (IOL) implantation in her left eye. She was treated by extraction of IOL and pars plana vitrectomy. Methicillin-resistant staphylococcus aureus, MRSA, was isolated from the aqueous, the posterior lens capsule and the vitreous. The condition subsided with favorable visual acuity after topical and systemic use of MRSA-sensitive antibiotics. The removed IOL showed numerous phagocytized staphylococ-ci by electron microscopy.
