We evaluated 30 eyes treated by penetrating keratoplasty regarding corneal topography and diffuse epithelial keratopathy. We paid particular attention to the corneal curve inner to the site of keratoplasty and to the epithelial damage of the transplanted cornea. The latter was graded into 4 stages according to the area (A) and density (D) of punctate epitheliopathy. The curve was significantly smaller in damaged donor cornea (A2, A3, D2, D3) than in eyes with no damage to the donor cornea (A0, D0) (p<0.01). Since a larger curve would better retain the tear fluid, the observed epithelial damage appeared to be secondary to the lesser corneal curvature.
