眼窩巨大類表皮嚢胞の1例

A case of giant epidermoid cyst in the orbit

　成人女性にみられた眼窩巨大類表皮嚢胞の1例を報告した。患者は22歳女性で，徐々に進行する右眼の下方偏位のため当科に紹介された。CTとMRI検査で眼窩上部に嚢胞状の腫瘍性病変を認めた。腫瘍へのアプローチ法の検討のため，術前にヘリカルスキャン法による3D-CTを行った。これにより従来のクレンライン法よりも骨切除範囲を拡大することができ，広い術野で腫瘍摘出術を行った。病理組織学的検査にて類表皮嚢胞と診断された。

A 22-year-old female presented with bulging of the right frontal bone and downward displace-ment of the eye globe of 2 years' duration. Axial and coronal sections of computed tomography (CT) showed a slightly inhomogeneous giant mass with a large excavation of the right orbital roof due to pressure erosion. Three-dimensional CT showed details of rearrangement of the orbital bone and thinning of frontal bone. These CT features facilitated planning of surgery. Biopsy of surgically removed specimen led to the diagnosis of epidermoid cyst of orbit.
