吸引生検が診断に有効だった眼窩炎性偽腫瘍の1例

Aspiration biopsy was useful in the diagnosis in a case of orbital inflammatory pseudotumor

　右眼に進行性の眼球運動障害および乳頭浮腫をきたし，悪性腫瘍との鑑別診断に吸引細胞診が有効であった眼窩炎性偽腫瘍の52歳女性の症例を報告する。以前に右眼瞼腫瘤の治療を受けていた。右眼に全方向性の眼球運動制限，うっ血乳頭，眼球突出が認められ，視力は0.1であった。画像では，眼窩内に充満した腫瘍が認められた。以上より，偽腫瘍，偽リンパ腫および悪性リンパ腫などのリンパ球系の腫瘍が最も疑われ，転移性の硬性癌などの可能性も否定できなかった。吸引細胞診により腫瘍は良性の腫瘍と診断され，さらに生検により眼窩炎性偽腫瘍と確診された。

A 52-year-old female presented with diplopia of 9 months' duration. Eye movement in her right eye was restricted in all directions. Ophthalmoscopy showed papilledema and choroidal folds in the right eye.Computed tomography and magnetic resonance imaging showed homogenous tumor mass filling the right orbit. We performed aspiratioan cytology with a needle and aspirator devised by us. Histological studies led to the diagnosis of inflammatory pseudotumor. This case illustrates the value of needle biopsy in the diagnosis of orbital tumors.
